Subject Access Request

Please complete the form below to request a copy of your medical record and/or test results from the practice. 

Every patient is entitled to have a copy, free of charge, of their medical record. Please do consider that processing these requests can take some time, as every page of a record needs to be checked by a GP before being released to the patient. Please note, additional copies will be charged at an appropriate rate depending on the size of the record. 

If there is something specific you are looking for, or a range of dates you need for any purpose, telling us this in advance would help. You do not have to give us the reason but sometimes that can help to find the notes you are looking for. 

Equally, if there are specific tests you need, Hospital Clinic letters, or consultation notes from the GP practice, if these can be specified, that also helps cut down on paper, admin and GP time, and cost.

Collection of Records - We will contact you to let you know your records are ready. If you wish to receive them electronically, this can be arranged by providing your e-mail address. You will be sent the records via Egress file transfer, which will send a link and unique code to allow access to the records. The email will come from the clinical inbox.
