Sick notes
If you’ve been off sick for more than 7 days
To request a sick note:
- If this is your first sick note for this period of illness, you must arrange a telephone call with a GP. PLEASE NOTE: Sick lines can be backdated, but not forward dated, so they can be issued after the day they are required to start from. Sick line requests do not need to be issued on the same day. Please accept the next routine appointment.
- fill out a sick note extension request form if you are looking to extend your existing sick line. A GP will process this within 5 working days OR they will ask you to make an appointment
- If a GP asks you make an appointment, the administration team will contact you to inform you of this, otherwise the sick line will be ready to collect after 5 working days
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
