Change Nominated Pharmacy

Complete the form below to change your nominated pharmacy for prescription collection. You can also choose to collect your prescription at the surgery. Please note, it is the pharmacy that is responsible for collecting prescriptions from the practice, not the practice delivering it to the pharmacy. Repeat prescriptions are ready in 48 hours AT THE PRACTICE and will therefore take additional time to be collected and dispensed by the nominated pharmacy. Please allow appropriate time for requests to be turned around. Thank you.

The following pharmacies collect daily from the practice: 

Carrick Knowe, Murrayfield (Western Corner), Rights (Roseburn), Well Stenhouse

Boots - Corstorphine, Craigleith, Gyle

Omnicare - Clermiston, Parkgrove, Springwell

The following pharmacies collect between 1 and 3 times a week

Apple (Dalry), Avante (Sainsbury), Barnton, East Craigs

Lindsay and Gilmour (Blackhall)

Boots - Comely Bank, Shandwick Place

ALL OTHER PHARMACIES ARE POSTAL AND PRE-PAID/STAMPED ENVELOPES MUST BE PROVIDED BY THE PHARMACY OR BY THE PATIENT
